Request Images and Medical Records

For electronic requests:

You can now share or request a copy of your medical images electronically via PowerShare. Once you have a PowerShare account set up, you can share imaging studies with doctors and specialists as needed, as well as upload prior studies to be shared with Radiology Regional. The PowerShare network is encrypted to ensure your privacy is protected and no one can see your medical information without your consent.

For CD requests:

To request a copy of your images onto a CD for pick up at Radiology Regional’s Medical Records Department or USPS, fill out and submit the Medical Records Request form below. If you have questions about the medical records request process, please contact: 239-275-3160. After submitting the form below, you should receive a confirmation email from “LucidHealth Smartsheet” (au********@**************************al.com) that your submission was successfully sent.

Please Note: You will be charged $5.00 per CD. The number of CDs needed will depend on the file size.
